A collaborative referral path for complex TMJ, airway, and sleep-breathing cases

What we do at Real Smile Dentistry

Our practice takes a physiologic, airway-centered approach. That means we look at how breathing, jaw position, muscle function, and oral health work together, then build a plan around what is really driving the patient’s complaints.

Dr. Hamid Nassery, DMD has more than 30 years of experience and is known for TMJ-focused care and whole-body dentistry that connects function, comfort, and long-term stability.

Top reasons doctors refer to Real Smile Dentistry

TMJ and TMD with functional symptoms

Common presentations include jaw tension, clicking or popping, limited opening, headaches, and migraine patterns related to joint and muscle dysfunction.

Neuromuscular anatomic orthotics for TMD patients

Cases where orthotic therapy may be appropriate to support jaw position, muscle balance, and symptom relief.

Mandibular advancement device or appliance therapy

For sleep-related breathing concerns where oral appliance therapy is part of the plan.

Man peacefully sleeping in bed, lying on his side with a relaxed expression.
Woman covering ears in bed while man snores loudly next to her, looking frustrated and unable to sleep.

Sleep breathing disorders and airway management

Adult and pediatric airway-focused evaluation and care, often tied to sleep quality, snoring, or mouth breathing.

Tongue tie and myofunctional support

Lingual and labial frenectomies, plus myofunctional therapy supported by certified, full-time myofunctional therapists across both locations.

Diagnostics and records we commonly use

  • Comprehensive oral exam
  • Airway and joint evaluation with patient co-discovery
  • Radiographic evaluation, including CBCT when indicated

This supports a clearer diagnosis and more predictable planning, especially in complex functional cases.

What to expect after you refer

We aim to make referrals feel easy for your office and clear for your patient.

Step 1: Intake and consultation

We review the referral concern, symptoms, and health history, then perform our exam and functional evaluation.

Step 2: Clear plan and patient education

Patients receive a detailed explanation of findings and recommended care. When appropriate, this includes appliance delivery, guidance for use, and supportive instructions.

Step 3: Follow-up schedule

Typical check-ins include a 2-week visit and a 3-month follow-up exam to monitor response and make adjustments.

Step 4: Ongoing care and documentation

If the patient joins our practice for general dentistry, we continue to document findings during routine cleanings and exams to track stability over time.

When a referral

might not be the best fit

During the initial consultation, each patient is assessed to determine what treatment is needed. If a case would be better served elsewhere, we will explain why and help guide the patient appropriately.

FAQs

What should we send with the referral?

The most helpful items are recent imaging (if available), a short summary of symptoms and timeline, relevant medical history, and any prior treatments tried. If you are not sure what to include, call and we will guide your team.

How do you keep referring offices in the loop?

We prioritize clear communication. After evaluation, we can share a summary of findings, the recommended plan, and follow-up timing so your office knows what is happening and why.

Do you treat both adults and children for airway concerns?

Yes. Our clinical approach includes airway-focused evaluation and therapy planning for adults and children when appropriate.

Do you use mandibular advancement devices for sleep-related breathing concerns?

Yes. Oral appliance therapy, including mandibular advancement devices, is one of the approaches we may use when it fits the patient’s presentation and goals.
